Mrs. Ann Marie Lopena, School Nurse, ext. 204 Incoming freshmen and all students new to St. Raymond must have a complete physical and immunization update as required by New York State Law and no student will be admitted without the required physical examination and completed immunization record. • If not provided earlier, students must submit his Medical Form and Immunization Card on Orientation Day in September. • Immunization records must indicate the day, month and year the vaccine was received. • Any student who is not completely immunized will be suspended from school, and his name will be forwarded to the Health Department. • A student planning to join intramural or interscholastic sports must have a current physical indicating that he is physically/medically able to participate. The school nurse (ext, 204) is available Monday thru Friday, 7:45am to 2:15pm. |
